* predefined_mci.js: General method for resolving MCI codes -> string

* Changed MCI resolution to use object to function mapping so entries not yet available would not be executed
* enigmaToAnsi() can now (optionally) resolve MCI codes
* pipeWrite() resolves MCI codes
